Since the 90s, Dhaka has experienced a two-fold increase in its population, making it one of most densely populated cities in the world. The 2014 census noted an abrupt surge in the number of slums in Dhaka as well as increased slum population, and almost a third of the total urban population lives in densly populated slums with poor living conditions. In most cases, residents do not have access to basic amenities, increasing the risk of communicable and non-communicable diseases.

Whilst a robust primary health care network exists in rural areas of Bangladesh, a strong primary health care network is absent in the rapidly expanding urban areas. As a consequence, urban residents seek expensive private health care services, unaffordable for the poorer urban population, and out of pocket expenditure is high, even if a free treatment plan has been obtained. The scacity of public primary healthcare and high costs of medicine and services is an additional challenge for poorer urban residents, who are already more vulnerable to poor health.
